A surgical guide is a custom-made template that fits over your teeth or gums during implant surgery. It contains precision sleeves that direct the drills to a position, angle and depth planned in advance on a computer. How it is made
Two datasets are combined: a cone beam CT scan showing bone volume and the position of critical structures such as the inferior alveolar nerve and the maxillary sinus, and a digital scan or impression of your teeth and gums. These are merged in planning software, where the ideal position of the final crown is decided first and the implant is then positioned to support it — restoration-driven planning rather than placing the implant wherever bone happens to be most convenient.
The guide is then 3D printed or milled to fit precisely.
Why it matters
Accurate placement is not just about safety, though avoiding the nerve and sinus obviously matters. It determines whether the final crown can be made with the right shape and emergence profile, whether it can be cleaned properly, and how evenly chewing forces are distributed. A poorly angled implant can be structurally sound yet produce a restoration that looks wrong or traps plaque.
Guided surgery also frequently allows a flapless approach, where the implant is placed through a small circular opening rather than lifting the gum. That typically means less swelling, less discomfort and faster healing.
What to expect
From your side, the guide adds a planning appointment and scans before surgery. On the day, the guide is seated and checked for a stable fit, then the implant is placed through it. Surgery is often shorter and more predictable than a freehand approach.
